Mental health office visit note

Teacher want us to complete the three areas of the note. This is an initial visit note just for psychiatric medication management. Please use the word client instead of him, her, his, she, he etc. Please do not put anything in note about medical stuff like diabetes or cancer. please use the exact file that is uploaded and must be completed by deadline.
